WAIVERS:
All rostered players must be on the team waiver. No need to print them – just fill them out online using the “Online Waiver” link below!
Click on “Team Name” and choose the team that applies. From the scroll down menu (High School and Middle School teams are listed separately), fill out the information needed and sign to complete.
Note: Parents signing for more than one athlete can do that by selecting “Add Another Name.”
Adult Note: Players who are over 18 years of age can sign for themselves as an “Adult.”
